Survey uncovers big disparities in patient satisfaction with GP practices

A Healthcare Commission survey published today concludes patients are generally satisfied with their GP practice – but there are major variations in trusts across the country.

The survey of 69,000 patients across England, conducted on behalf of the Healthcare Commission by the Picker Institute Europe, found patients ‘increasingly positive’ about the care they receive.

74% said the main reason they went to see their GP was dealt with ‘completely’ to their satisfaction, up from 73% in 2005, while 77% said they ‘definitely’ had trust and confidence in their doctor, up from 76%.

But the survey, which breaks down patient satisfaction by PCT, also uncovered ‘significant’ disparities in satisfaction across the country. While overall 75% of patients said they had waited two days or less for their last GP appointment, in Torbay, this figure was 89%, while in Tower Hamlets, it was just 43%.

Anna Walker, chief executive of the Healthcare Commission, said: ‘This survey shows the high regard that many patients have for the services at their GP surgeries. That is really good news.’

‘However, people clearly do want to be able to see a GP more easily and at more convenient times. It was striking that some people could not get an appointment within two days and that there are variations across the country.’

The survey also highlighted rising concern over access to GP services. 25% of respondents said they had at some time been put off going to their GP because of inconvenient opening times, up from 21% in 2005. Younger patients were even more dissatisfied, with this figure rising to 38% among those aged 16 to 35.

Dr Richard Vautrey, GPC deputy chair, said: ‘There are variations with access across the country and we want all patients to receive a good service from their GP. Whatever their current rating, all practices with the support of their local health trust will want to further improve the service they provide to their patients.’

But overall, he added, the survey’s findings were very welcome.

‘With a limited number of appointments in any one day practices try very hard to strike a balance between making sure patients who want to book ahead are able to do so while still ensuring there are enough empty appointments for emergencies: on the whole it seems GP surgeries are getting it right,’ he said.

David Stout, director of the NHS Confederation’s Primary Care Trust Network, said: ‘The results of the survey confirm that there are generally very high levels of satisfaction with primary care and evidence of continued improvement.’

‘However it is clear that there is still quite wide variation in rates of satisfaction with access to both GP services and dentistry across the country.’

‘In the parts of the country where patients find it more difficult to get a GP appointment, PCTs will continue working hard to find local solutions to widen access, and the introduction of new health centres and extending opening will help improve access and patient choice.’

Go-ahead for Canary Wharf rail link with Heathrow

In a recent significant move, Lords have sanctioned the permission for a £16 billion railway link connecting Canary Wharf and London Heathrow Airport.

Most notably, the connection that has been proposed now will form part of the most welcomed Crossrail project. Notably, this is one of the largest development projects in the transport sector of London during the past 50 years.

On its completion, the present proposal is expected to significantly ease the congestion on the tube network in addition to the dampening calls for more services at the London City Airport, which is situated very close to Canary Wharf.

Since the Parliament’s upper chamber has approved the proposed link even without the requirement for a vote, the present move has clearly paved the way for the Crossrail plans to be brought before the House of Commons.

The construction has been planned to commence in 2010. If the construction takes off as per the schedule, the link will require not less than 14 miles of tunneling. The project will connect Heathrow and Maidenhead to east London through Paddington and Tottenham Court Road.

In a statement made recently, Ruth Kelly, the transport secretary had expressed her full support for the Crossrail project. She observed, “This landmark project is of major significance to both London and the whole country. It will generate jobs and economic growth, help re-vitalise some of our most deprived areas and deliver major improvements for the travelling public.”

RUTH KELLY: Crossrail hole plot agenda-courier cabinet minister herself.

Ruth Kelly, who is the official face of the CRASSrail hole-plotter UK central Govt  Department ‘for’ [?????] Transport [=DfT] is allegedly a resident in Tower Hamlets. In fact, the alleged reports suggest that she may be a resident in a Tower Hamlets borough  council ward that is not too far from Stepney…Or Brick Lane… Or Whitechapel…! May be she even has an address close to Wapping!

Then there is Jim F Patrick! He is a ‘sitting’ MP for one of the two UK House of Commons parliamentary seats allegedly representing Tower Hamlets.

Jim F Patrick too is a ‘minister’ in the same DfT!

Then there is Tom McNulty, currently assaulting civil liberties and the magna carta under cover of terrorism-countering legislation and promoting the 42 day detention without charge excesses. McNulty was at the DfT in 2004-2005 when the ‘Crossrail Bill’ was being drafted for formal introduction into the  UK House of Commons [22 February 2005] . McNulty is not known to have actually lived in Tower Hamlets AS SUCH. But he has had access to the most intimate [in the sense of PROXIMATE and exclusive] of sources to get to find out about Tower Hamlets. Whatever the realities may be of his marriage to Christine Gilbert, the fact that McNulty was married to Christine Gilbert during the time that she was employed in Tower Hamlets Council gave McNulty a very important and practical source of information about the East London Borough… 

KHOODEELAAR! made the discovery of the fact that McNulty was married to Gilbert in the course of the KHOODEELAAR! constitutional law research and investigation in 2004  

Immediately, we put a series of legal and constitutional law questions to Christine Gilbert and asked her as delicately, diplomatically and ethically as we could whether she was breaking her duties to the community and to the electorate in any way over her part as the Council’s chief executive in relation to the Council’s inviting of the Crossrail hole plot attacks on the community and the East End of London….. We specifically asked her about the status, the implications of her advice…

By advice, we meant advice to the ‘legally constituted d entity of the Council’ . 

The context was her relationship with McNulty. McNulty was not just a minister in the DfT. 

He was very closely involved in the DfT’s dealings with the CROSSRAIL scam itself.

To this day, we have not received any answer from Christine Gilbert to our questions. 

Just as we have not received any answer

We are stating  this for the first time here on Monday 28 July 2008 because  the time ha snow come for all the legal acton notices we have been issuing to be given practical application. Christine Gilbert is definitely included as a defendant in the constitutional law actions… We shall have lots more on her role in INVITING the Crossrail hole plot attack on the East End of London….

We point out that in the first half of the piece, there is crazy, in the sense of irrational language sued to describe some aspects of the delay  before the scam became ‘ACT’ of Parliament [22 July 2008].. We also draw attention to the fact that the concept of BUSINESS is stressed as the source of the demand for CRASSrail. The FT wholly identifies with ‘Business’. Ambitious rail plan finally leaves sidings

The patience of business has been sorely tested during its elephantine gestation period.

But Crossrail, the £15.9bn rail project to link Heathrow directly with London’s financial centre, finally reached the end of a tortuous 20-year journey to secure royal assent yesterday – holding out the promise of a transformation in the corporate travel experience within a decade.

The rail link, which will be 118.5km long and carry 200m passengers a year, is designed to lop at least a third off the journey time between Heathrow and Canary Wharf.

Work will begin in spring or summer next year to clear the way for construction to start in 2010. The new link is expected to start operating in 2017 and the entire route would be open within 12 months, said Crossrail.

The ambitious rail plan has been in the making since the late 1980s but was shelved in the early 1990s when a recession forced the government to rein in public spending.

But lessons have been learnt from what is now seen as the premature decision to abandon it when times got tough. With polls suggesting the Conservatives may win power before the scheme is completed, a cross-party consensus ensures it will survive whatever the government’s stripe. Crossrail said all parties shared the view that, in spite of the economic slump,the long-term benefits and the scheme’s wider contribution to the economy must override short-term concerns about the health of the public finances.

Crossrail, which becomes a wholly owned subsidiary of Transport for London from today, will recruit a chief executive to take over from Douglas Oakervee, the current executive chairman. Four non-executive directors were yesterday appointed to the board. They are Michael Cassidy, Patrick Crawford, Sir Joe Dwyer and Heather Rabbatts. Mr Cassidy is an independent non-executive director at UBS. Mr Crawford is chief executive of the Export Credits Guarantee Department in London, Sir Joe is the former president of the Chartered Institute of Building and the Institution of Civil Engineers. Ms Rabbatts is a former governor of the London School of -Economics.

The scheme, expected to cost £15.9bn, will be financed by a £5.1bn grant from the Department for Transport; £8.2bn from the City of London; and £500m from the City of London and BAA, the airports company that owns Heathrow. The rest will come from a variety of sources, including the London Planning charge, a controversial levy that will be paid by London businesses.

The direct gains fromEurope’s largest infrastructure project, in-clude up to 30,000 new jobs, with 14,000 people employed full-time at the peak of construction in 2013-14; commercial, retail and residential property developments; and greater access to the City for people who live in the south-east.

The City of London Corporation believes Crosslink will play a critical role in helping London maintain its status as the nucleus of the world’s financial industry. Stuart Fraser, policy chairman told the FT good transport infrastructure was essential, with competition intensifying to seduce the world’s biggest players away from London to European capitals, such as Paris, and emerging financial hubs in the east, such as Shanghai.

“We don’t have a choice. There is a temptation when government budgets become strained to take a short-term view and push major projects into the long grass until times improve. Thank goodness the Victorians didn’t take a short-term view, otherwise we would never have had the Underground,” he said.

Mr Fraser said, however, that Crossrail would only be as strong as its weakest link, which made it crucial that other aspects of London’s transport infrastructure were up to the same standard.

How travelling will become easier

*State-of-the-art trains will have extra-wide doors to help passengers with luggage

*Wheelchair access at seven new stations

*The 600 carriages will have air-conditioning, and potentially additional features such as wi-fi and mobile phone reception

*Travellers will journey directly from Heathrow to Abbey Wood in 56 minutes. The trip currently involves at least two train changes and takes about 90 minutes

*Adding four trains an hour on the line between Heathrow and Paddington used by the Heathrow Express services, which also runs four times an hour, will ease congestion on the route

*Crossrail will be fully integrated with Transport for London, which means commuters will be able to use their Oyster payment cards – no supplementary fare will be required

*The fare will be significantly cheaper than the Heathrow Express, which costs up to £23.50 one way

*A dedicated Crossrail station will be built at Heathrow by BAA, the airports authority

*Canary Wharf Group will build its own facility

That it is not the right answer to the EXISTING transport needs of London we have said so since the campaign  against Crossrail hole agenda attacks on the East End of London was begun  on 31 January 2004… We have said it in a thousand article, files, dispatches, diagnostic items ALL published on the internet and on more than 100 separate web sites an d web links and blogging sites  ways…

 

 In  over 10 millions  documentary, analytical, investigative words we have authored, created and published over 4 years and six months of daily research, analyses and publishing against the Crossrail hole scam….! 

 

 

We have said it in print. We have aid it verbally. we have said in  letters, statements  and comments… Not a single other publication, or source has said so. Not anywhere nearly as clearly, consistently, coherently as they have now begun to say! It is very significant that the Daily Telegraph[ah dated  23 July 2008 says nearly as many key things about the defects of Crossrail as we have been saying . Why is that date significant? Because it is the day after we published [and served on Johnson ] our comprehensive legal action programme and ground s against Boris Johnson [on the basis of his refusal up to 22 July 2008 to disown Ken Livingstone's 'mayoral' 'commitment' to Crassrail]…..

 

We are vindicated… again…. 

 

 

But that is not enough… 

 

We want Boris  Johnson-friendly Daily Telegraph [and Boris Johnson himself] to go all the logical and responsible way and say everything that Khoodeelaar!! has said for the past 56 months… with the evidence that Khoodeelaar! has unearthed, enlisted and published showing the unsuitability, the undesirability and the crassness and wastefulness and the diversion of Crossrail…… [To be continued]

Crossrail has a tough journey ahead

So much energy has been expended on Crossrail that anyone would think it had been built already. No such luck.

After the latest three-year debating bout, the Bill for the cross-London rail project has finally won Royal Assent. As Stuart Fraser put it: “Now we come to the tough bit. The building of it and the funding of it.”

Fraser is the chairman of policy at the City of London Corporation, which has been lobbying like crazy for the £16bn rail link, due to be operational in 2017.

It will run from Maidenhead and Heathrow in the west through two tunnels under central London to link Shenfield and Abbey Wood in the east. New stations are planned at Paddington, Bond Street, Tottenham Court Road, Farringdon, Liverpool Street, Whitechapel and the Isle of Dogs.

An extra 1.5m people will suddenly be within 60 minutes’ commuting distance of London’s main business districts, with Heathrow a mere 31-minute trip from the West End and just 43 minutes from Canary Wharf. All up, the project is forecast to create 30,000 jobs and add at least £20bn to the UK economy.

“A cracking deal for the capital” is how London Mayor Boris Johnson described a project first proposed in 1989 under a Conservative government. That’s assuming the funds really materialise.

  • The Department for Transport’s press release noted that Gordon Brown had announced last October that “a £16bn funding package had been secured” to build Crossrail. How secure is a moot point.

    Says Tony Travers, of the London School of Economics: “We have got to the end of the parliamentary process, but I would still say the money is some way from being in the bag.”

    The DfT is putting up £5.1bn courtesy of the taxpayer, but the rest is less certain. Mayor Boris, via the Greater London Authority and Transport for London, is on the hook for £7.7bn, a big chunk of which still requires legislation. The City of London has promised to cough up £350m – though only £200m is guaranteed – while other companies, including Network Rail and the cash-strapped BAA, must contribute.

    It is the Mayor’s chunk where there are most moving parts. Some £3.5bn is due to come from a business rates supplement of 2p per pound, expected from April 2010. The Mayor is also banking on a £300m contribution from property developers around Crossrail stations and £300m more from a new statutory planning charge, also scheduled for April 2010.

    Then there’s a £2.7bn contribution from TfL, which comes out of its £38bn 10-year funding settlement to March 2017. The worry is that, after the collapse of Tube maintenance company Metronet, TfL could be called to plug further funding gaps on London Underground.

    Travers reckons that repricing the Tube contracts signed under the public private partnership deal, including those held by Tube Lines, could add £5bn more to TfL’s bill, straining its resources for Crossrail.

    Fraser says: “The one thing we have to keep an eye on in this project is there is no robbing Peter to pay Paul. It would be no good having a superb Crossrail coming into a creaking Underground network.” That’s not to mention the risks of further hikes to the cost of the 2012 Olympics, which may also come out of Boris’s budget.

    Moreover, as Travers points out, all these different sources of finance for Crossrail “have to be brought together at a time when raising money for big projects is not the easiest thing to do”.

    All this before the risks of cost overruns in building the rail project. Which is why Royal Assent may only be the start of the Crossrail journey.
